The Birth Codex is a permanent, custom-produced archival document. It is not a digital product, a subscription, or a novelty item. Once produced, it cannot be recreated. Please read these terms carefully before submitting a Provenance request.
1. About the Service
My Birth Codex™ ("the Company," "we," "us") is a birth identity infrastructure company headquartered in the United States. We produce the Birth Codex — a permanent, archival-grade identity record issued at birth — and operate related services including the Codex Companion, Codex Brief, MYA digital registry, Codex Core hospital infrastructure, and Codex Certified professional credentialing program.
These Terms of Service ("Terms") govern all use of our website at mybirthcodex.com, submission of a Provenance request, purchase of any product or service, and access to MYA or any digital service. By engaging with any of the above, you agree to be bound by these Terms in full.
2. Eligibility
To submit a Provenance request or purchase any product or service from My Birth Codex™, you must be at least 18 years of age and legally authorized to act on behalf of the child being recorded. By placing an order, you represent that:
- You are at least 18 years old
- You are the parent, legal guardian, or an authorized representative of the child named in the record
- All information you provide is accurate and complete to the best of your knowledge
- You have the legal authority to commission a permanent identity record on behalf of the named child
Orders placed on behalf of a child by someone without legal authority to do so are void and will be subject to cancellation without refund.
3. The Birth Codex Record
What It Is
The Birth Codex is a custom-produced, museum-grade archival document printed on 100-pound cotton-rag paper with a minimum 500-year archival life. It records 23 or more dimensions of a child's identity at birth — including clinical data, celestial alignment, numerological profile, lineage, and physical measurements — and is produced as a one-time, permanent record.
What It Is Not
The Birth Codex is not a legal identity document. It does not replace or supersede a government-issued birth certificate, passport, or any other official document. It is an archival identity record — privately issued, permanently produced, and intended to complement, not replace, official documentation.
Record Accuracy
The accuracy of the Birth Codex record depends on the accuracy of the information provided at the time of the Provenance submission. My Birth Codex™ produces the record based solely on the information submitted. We are not responsible for errors resulting from incorrect, incomplete, or fraudulent information provided by the ordering party.
If a factual error is discovered in the record that is attributable to a production error on our part, contact us within 30 days of delivery. We will assess the error and, if confirmed, produce a corrected record at no additional charge.
4. Orders & Payment
Provenance Submission
All Birth Codex orders begin with a Provenance request — a structured intake process at /provenance/ in which the ordering party provides the identity and birth data required to produce the record. Submission of the Provenance form constitutes a binding order and payment obligation.
Pricing
Birth Codex pricing is tiered by coverage level:
- Origin — $247
- Archive — $497
- Legacy — $897
Codex Sovereign pricing is separate and governed by a private agreement. All prices are in United States dollars and are subject to change. The price at the time of your Provenance submission is the price applicable to your order.
Payment Processing
Payment is collected at the time of Provenance submission. We use PCI-compliant third-party payment processors. By submitting payment, you authorize us to charge the stated amount to your designated payment method.
5. Refund Policy
The Birth Codex is a custom-produced archival document. Production begins within 48 hours of Provenance submission. Because each record is produced uniquely for a specific child and cannot be resold or repurposed, the following refund policy applies:
- Before production begins: Full refund available within 24 hours of submission. Contact us immediately at hello@mybirthcodex.com.
- After production begins: No refund is available. The record is in production and is being produced exclusively for the named child.
- After delivery: No refund. The record has been fulfilled.
Exceptions are made only in cases of a confirmed production error attributable to My Birth Codex™. In such cases, we will produce a corrected record at no charge. We do not offer refunds on the basis of dissatisfaction with the content of a correctly produced record.
6. MYA Subscription
Access & Eligibility
MYA (Moment You Arrived) is available exclusively to families who hold a verified Birth Codex record. MYA is not available as a standalone service. Access is granted upon verification of an existing Codex record.
Subscription Terms
MYA is offered as an annual subscription at the following tiers:
- $111 per year — Standard access
- $222 per year — Enhanced access
- $333 per year — Full access
Subscriptions renew automatically at the end of each annual term unless cancelled. You may cancel at any time before the renewal date. Cancellation takes effect at the end of the current billing period — no partial-period refunds are issued.
Service Continuity
My Birth Codex™ commits to maintaining the MYA registry and its data for the life of the account and beyond. In the event of any discontinuation of the MYA service, we will provide no fewer than 90 days' notice and a full data export to all active account holders.
7. Institutional Use — Codex Core & Codex Certified
Codex Core (Hospital Systems)
Codex Core is an institutional service available to hospitals, health systems, and licensed clinical facilities. Use of Codex Core is governed by a separate Master Services Agreement and Business Associate Agreement (BAA) executed between My Birth Codex™ and the institution. These Terms of Service apply to website access and inquiry only — not to the institutional service itself.
Codex Certified (Professionals)
The Codex Certified credentialing program is available to qualifying birth professionals — doulas, independent midwives, and licensed birthing center operators. Certification is subject to a separate agreement governing use of the Codex Certified credential, Codex Brief fulfillment, and professional conduct standards. Certification may be revoked for conduct inconsistent with those standards.
8. Intellectual Property
Our System
My Birth Codex™, Birth Codex, Codex Core, Codex Companion, Codex Brief, Codex Sovereign, MYA, Moment You Arrived, Codex Certified, and all associated logos, marks, document designs, system architecture, and content are the exclusive intellectual property of My Birth Codex™ and its founders. All rights reserved.
Your Record
The physical Birth Codex document delivered to you is your property. You own the physical artifact. The underlying document design, system architecture, authentication methodology, and production process remain the intellectual property of My Birth Codex™. You may not reproduce, duplicate, or commercially exploit the document design for any purpose.
Content You Provide
You retain ownership of the birth data and supplemental information you provide through the Provenance process and MYA. By submitting this information, you grant My Birth Codex™ a limited, non-transferable license to use it for the sole purpose of producing and maintaining your record.
9. Prohibited Use
You agree not to use My Birth Codex™ services to:
- Submit false, fraudulent, or fabricated birth data
- Commission a record without legal authority to represent the named child
- Reproduce, resell, or commercially exploit the Birth Codex document design or system
- Circumvent, reverse-engineer, or interfere with any part of the Codex production or authentication system
- Use the Birth Codex as a fraudulent substitute for a government-issued identity document
- Engage in any activity that violates applicable law or the rights of any third party
Violation of these prohibitions may result in immediate cancellation of your order or account, forfeiture of any fees paid, and referral to applicable authorities.
10. Disclaimer & Limitation of Liability
The Birth Codex is provided as a permanent identity record produced from the information you submit. My Birth Codex™ makes no representation that the record constitutes legal proof of identity for any official or governmental purpose.
To the fullest extent permitted by law, My Birth Codex™ shall not be liable for any indirect, incidental, special, consequential, or punitive damages arising from your use of any service, including but not limited to: errors in data submitted by the ordering party, loss of the physical document after delivery, or any third-party reliance on the record.
Our total liability for any claim arising from a Birth Codex order shall not exceed the amount paid for that order.
11. Governing Law
These Terms are governed by and construed in accordance with the laws of the State of Delaware, without regard to its conflict of law provisions. Any dispute arising under these Terms shall be resolved exclusively in the state or federal courts located in Delaware, and you consent to the personal jurisdiction of those courts.
If any provision of these Terms is found to be unenforceable, the remaining provisions shall remain in full force and effect.
My Birth Codex™ reserves the right to modify these Terms at any time. Material changes will be posted at mybirthcodex.com/terms with a revised effective date. Continued use of any service after such changes constitutes acceptance of the revised Terms.
